It has been noted that many of the beneficiaries are often unaware of the documents required for new card registration and for addition of family members to existing cards, leading to delays and repeated visits to healthcare facilities. The DLSA has therefore appealed to the public and eligible beneficiaries to keep the necessary documents ready in advance.

For new enrolment, beneficiaries may carry Aadhaar Card (mandatory) along with supporting documents such as Voter ID Card, Birth Certificate, MGNREGA Job Card, Ration Card, or any other government-issued photo identity document. Family-related documents may include Ration Card, RSBY/MHIS Card, Prime Minister's Letter (where applicable), government records containing family details, or a Headman's Declaration.

For addition of family members to existing PMJAY cards, supporting documents such as Birth Certificate, Adoption Certificate, Marriage Certificate, Ration Card, or Headman's Declaration may be produced, as applicable.

The DLSA has urged all concerned to ensure that the required documents are readily available to facilitate smooth and hassle-free enrolment and updating of AB-PMJAY/MHIS cards during the Mega Camp
